Placement Partners is happy to announce the completion of provider enrollment for Day Support Services (DSS) and is now accepting referrals. Unlike a traditional in-center setting, our DSS program will serve individuals solely in their local community on a one-to-one basis. The DSS program will provide individualized assessment and planning to help a person identify essential life skills, needs, and discover activities that enrich their lives. Some examples of those life skills may be communication, budgeting, problem solving, goal-setting and socialization.
Since the transition from Housing Access Coordination (HAC) to Housing Stabilization Services (HSS) took place in August 2020, Placement Partners has quickly expanded our housing program capacity to meet the needs of the community we serve. Currently, our HSS program serves almost 200 individuals and we look forward to serving more individuals that are in need of relocation.
We are accepting referrals for both programs in the seven major counties across the Twin Cities area (Hennepin, Dakota, Anoka, Ramsey, Washington, Scott, Carver). Referrals may be completed online. Upon completing the referral form and submitting the referral supporting documents, our corresponding Program Director will reach out to you to start the intake process.
