Employment Services

A federal, state and county funded program supporting waivered and non-waivered individuals with disabilities by securing and maintaining local competitive employment at or above minimum wage.

Home-Based Services

A federal and state funded program supporting waivered and non-waivered individuals with disabilities by developing, maintaining and improving skills needed to successfully live independently in the community.

Housing Stabilization Services

A Minnesota Medical Assistance benefit to help people with disabilities, including mental illness and substance use disorder, and seniors find and keep housing.

Case Management Services

A service that provides a person and his/her family with access to assessment, person-centered planning, referral, linkage, support plan meeting, coordination and advocacy related to waiver services, resources and informal supports that are not necessarily funded through the waiver.

Placement Partners is a family owned Minnesota business that has been assisting people with disabilities since 2004. We are results oriented and committed to provide person centered Vocational, Residential and Case Management services throughout the seven county metropolitan area. Each year we provide these services to hundreds of adults striving to achieve a greater level of self-reliance.

Our Mission

The delivery of person centered services supported by effective strategies that include advocacy for inclusion within the workplace and skills training that promote independent living shall be the standard by which we are measured.

How We Work

Our Process

Referral.  Upon completing the referral form, we will reach out to you to ask questions and to set up an intake and / or information meeting.
Intake.  We will explain the program in depth and answer all of your questions. Upon acceptance, all required paperwork is completed at this meeting and this concludes this phase of the process.
Discovery.  Our trained staff will meet with the individual to learn about their wants and needs. Corresponding assessments are completed in helping our staff to better serve the individual.
Assistance & Training.  From job search to skills training, we will help you along the way to achieve the goals established at the intake meeting.
On-going Support.  We will be there to support your program goals whether it be securing a job and maintaining employment or developing the skills to live independently. Progress is measured and results are discussed at team meetings. You goals may be re-evaluated and new goals established to support desired outcomes.

1. Referral

To gather the needed documentation and to ask the right questions prior to info/intake meeting.

2. Intake

To answer all questions in person and collect the needed signatures for required documents per the Department of Human Services.

3. Discovery

To complete one or more informal sessions to understand the individuals needs and wants.

4. Assistance & Training

To learn vocational community-based new skills or to receive support in person.

5. On-going Support

To meet on a weekly basis to ensure ongoing, continuous support in achieving predetermined goals and objectives.
